Hospital-Based Transitions of Care Services: Pharmacy Models in the Americas

Pharmacists play a pivotal role in preventing medication-related harm at transitions of care. This webinar will describe hospital-based pharmacy services that go beyond medication reconciliation to ensure safety and continuity of care for patients. Experts from Canada and Costa Rica will describe unique models that incorporate medication management, opioid and antimicrobial stewardship, and caregiver education to deliver the highest quality transitions of care services.

1. Identify risks and challenges during care transitions, drawn from literature and clinical experience, that compromise patient safety
2. Determine the roles and responsibilities of pharmacists, according to their practice setting, in ensuring patient safety across the continuum of care
3. Illustrate how hospital pharmacists can lead practical, scalable transitions-of-care services that improve medication safety and support continuity of care across healthcare settings.
